This position supports low-income families by providing culturally relevant, strength-based services such as education, advocacy, case management, and resource linkage.
Key Responsibilities
- Plan and implement outreach strategies and coordinate services with community partners.
- Conduct intake, assessments, and develop service plans collaboratively with families.
- Provide case management for 13-15 families annually, supporting their goals and needs.
- Assist families in accessing community resources, monitor progress, and facilitate referrals.
- Offer individual and group support, including workshops and support groups, both in-person and virtually.
- Attend meetings, facilitate outreach, and maintain documentation and data records.
- Collaborate with mental health professionals and participate in training for professional growth.
- Promote a positive work environment, demonstrate professionalism, and adhere to confidentiality standards.
- BA in Social Work or related field, or equivalent experience.
- One year of case management or related experience.
- Experience working with Latino immigrant populations and facilitation of workshops or support groups.
- Strong organizational, problem-solving, and communication skills.
- Bilingual in Spanish and English (reading, writing, speaking).
- Ability to work evenings and weekends, travel as needed, and pass background and health clearances.
